Center for Retina and Macular Disease looks forward to having you join our host of satisfied patients. Every member of our team -- physicians, medical assistants, technicians, secretaries, receptionists, aides, and insurance specialists -- is committed to you and your family. Each of our doctors has schedules tailored to your needs. CRMD is available when you need us.
CRMD is open from 8 AM to 5 PM, Monday through Friday. Our offices are closed on New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day.
Once you have made your initial appointment, you can download and complete our registration forms by simply clicking on the links below. You can bring the completed forms to your first appointment. This will help to speed up the registration process and get you in to see the doctor faster.

- List of medications
- Insurance ID card
- Any appropriate insurance referral authorizations
