Low Vision Expertise
In addition, we have a Low Vision optometrist to assist when ordinary eyeglasses, contact lenses, and surgical procedures cannot provide adequate vision to perform everyday activities.

We participate in numerous clinical trials. Patients who qualify for a clinical trial are offered cutting-edge technology at no cost. For more information, inquire during your first appointment.
What to Expect
Every member of our team -- physicians, medical assistants, technicians, secretaries, receptionists, and insurance specialists -- is committed to you and your family. Each of our doctors has schedules tailored to your needs. CRMD is available when you need us.
CRMD is open from 8 AM to 5 PM, Monday through Friday. Our offices are closed on New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day.
Your First Appointment
You may request an appointment online via our Make An Appointment page or call us at 863.297.5400, ext. 5005. Once your appointment has been made, download and complete our registration forms by simply clicking on the links below. You can bring the completed forms to your first appointment. This will help to speed up the registration process and get you in to see the doctor faster.
