What to Expect

Every member of our team – physicians, medical assistants, technicians, secretaries, receptionists and insurance specialists – is committed to you and your entire family.  Each of our doctors has schedules tailored to your needs.  CRMD is available when you need us.

CRMD is open from 8 AM to 5 PM, Monday through Friday.  Our offices are closed on New Year’s Day,  Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day.

Your First Appointment

You may request an appointment online or call us at (863) 297-5400, ext. 5001.  Once your appointment has been made, please download and complete (Spanish)our registration forms.  You can bring the completed forms to your first appointment.  This will help to speed up the registration process and get you in to see the doctor faster.

Each time you come to the office, please bring your current:

• List of medications
• Insurance ID card
• Driver’s license or other government-issued photo ID
• Any appropriate insurance referral authorizations
• Co-payment, expected at time of service
